The icons for files change to the wrong thing... and now, I'm seeing the wrong icon in the window when I connect to a server.
Is there a utility to fix this?

hey try repairing permissions in the disk utilily. i had the same problem and this seemed to help.
First I tried Repairing Permissions w/ Disk Utility (the obvious)
Then I tried Tech Tool Pro 4 file repair... but it passed all the tests...
This is just weird...
Apparently, this is an issue with the 10.3.8 update...
but I believe it is related to the intermittant problem I was having before:

What was happening recently is that icons would appear strange all over the place, for documents, or it would be a completely wrong icon, an icon for something else, for some applications, or even the network alias would have the wrong icon (an icon of some random app, that would appear all throughout the connect to servers dialogue box).
this seems to take care of the problem
remove ~/Library/Preferences/com.apple.finder.plist,
then force-quit the Finder.
