I've been looking around various sources for docs for the ALS Smarterm II 80-column card but haven't had any luck. Does anyone happen to have a copy for sale or one that I could borrow to scan? The Videx card that came with my Apple II+ is dead right now and I'd like to setup the Smarterm card until I have the Videx working.

I have the card, and at least used to have the documentation for it, although I have no idea where it is at the moment.
What do you need to know about the card? For the most part it is just plug it into slot 3 and attach the short cable to the back of the ][+ (or clone) and the monitor cable to the other connector. The card has the "soft switch" functionality internal. If you want to use it in BASIC, it is pretty similar to the Videx... PR#3. Other software will have to support the SmartTerm and/or have a SmartTerm pre-boot. What are you wanting to use that needs 80 columns? Unfortunately some software only supports Videx.
I was curious about the pin header and the jumper. I figured it would just work in Slot #3 without much issue but I hadn't found much info about it yet. I think the RCA jacks are pretty much junk on the Videx so was waiting on a couple of new ones to replace them. Thanks for the clues! I'll probably put it in the machine tomorrow and give it a shot.
