first welxome to the platform.
Answer to the given question:
The card is one of the versions of the FDC4.
The original card was made and distributed by Ehring.
It´s a floppy controller card that offers the chance to connect
either 2 Apple II DISK II drives or
2 drives with shugart bus ( 34 pin floppy cable ).
At this card the logo from Ehring is missing and the layout changed a slight bit
so it´s a clone.

So I can use that CTRL with any low density 5.25 floppy drives? Does it also support 3.5 drive?

in general that controller also can use lowdensity 5,25 drives
with eiter 40 or 80 tracks and single-sided or doublesided.
But in fact that controller does not work very well with "normal" drives.
In former days in Germany that controller was used with the TEAC 55-F
drives and that drive was slightly modified for better cooperation with
that controller.
I have the description of that kind of modification somewhere within the dungeons
of my archive. But at the moment i haven´t discovered it again and therfor that
description is still missing in my site.
If i have luck that document will be found within this summer and added to the site.
In that case i will update my reply here with a link to that page.
