YKH-128 (aka HMS-3264) EPROM Programmer... searching for the software disk

I have this old EPROM Programmer card (called a  YKH-128) that I acquired around 1983.  It is the type with three ZIF sockets designed to handle 2716, 2732, 2732A, and 2764/27128 respectively with the correct volatges generated for burning reliably. It is also an almost exact clone of the Hollister Microsystems HMS-3264 card. Over the years and several moves, I no longer have the software diskette for driving it, and the disk image does not appear in the usual repositories (eg Asimov).  No roms on this card, all of the logic is driven from the utility program. 

Does anyone have the disk or can provide an image that I can use with this card? It is different software than is used with the common AP-64 type (single socket) EPROM card.

Thank you very much!! Still searching for the HMS-3264 or YKH-128 software disk to drive this card. Have kept an eye out on Asimov and other archives but none have showed up yet. A disk was sold on Ebay about 12 months+ ago but was snapped up before I saw it. Hopefully someday a fellow applefrittarian has it in their disk stash awaiting archiving.

More than 36K members is no joke and I have seen them deliver in similar situations.
