It looks to me like a Floppy Disk Drive Controller Card.
It has two connectors in the center for two seaparate drives.

as far as I know you can connect two Apple Disk ][ drives to this floppy controller.
In addition to this the controller has a 34pin header for PC 5 1/4" disk drives like the TEAC FD-55.
PS: Do you have the patchware disk to modify DOS3.3 or PRODOS to be able to access the 40/80 track disks?

the erphi controller is a controller for APPLE DISK II Floppydrives and for industrial floppydrives 80 track 2 sided with so called shugart bus ( the usual 34 pin Floppycable ).
I´ve uploaded the entire manual of the erphi controller AFDC2 ( AFDC2 is the correct typename ) in german language to my website.
It includes detailed description of the switches in a table-graph and detailed circuitplan of the controller
I also added the detailed manual of the Ehring FDC4 Controller and the description of the patching software PATCH165 in german language
i also uploaded the circuitplan of the controller....
in case that you can´t identify the floppy controller mail an photoimage of the controller to me... i´got a database with some 25 to 30 different floppycontrollers and i might be able to identify the controller and provide the information via my downloadpage.

besides I´ve also got for most of the controllers also the correct eprom-images and different special versions....

I´ve just taken a look to the posted image and that image (link in the first posting ) shows up a Ehring FDC4.
That is a floppycontroller too. it supports too 2 DISK II Drives or 2 industrial drives with up to 80 tracks and
2 sides on 34 pin floppycable. But that controller does need special patching software ( called Patch165 ). I already explained in the upper posting that I uploaded the manual, the circuitplan and the patchdisk as .dsk file for ADT to my site ( see for adress of the site the posting upward.. ).
